Successful Spine Surgery can relieve debilitating pain even in very Old patients (Geriatric Patients) – Dr Sandeep Moolchandani

Yes, successful spine surgery can indeed relieve pain in geriatric patients, even those who are older in age. While age is a factor to consider when evaluating the risks and benefits of surgery, modern medical techniques and advancements have made it possible for older individuals to undergo spine surgery with good outcomes.

This patient had Lumbar canal stenosis L3 to L5 with severely degenerated spine with Disc Prolapse L45 and L5S1 and osteoporotic compression fractures at multiple levels. Patient was bed bound and she could barely walk 50metres without pain. At her age of 68 years, the family was loosing hope for any successful treatment.

They came to us and she was operated. During surgery L5 Laminectomy, L45 Discectomy and Bilateral Foraminal decompression L45 L5S1 was done. Patients pain was relieved immediately after surgery and she was discharged from hospital after 3 days. For osteoporotic fractures she was put on treatment with synthetic form of natural human hormone called parathyroid hormone (PTH) to make her bones stronger. Now the patient is able to walk without any pain and she has found a new hope in her life by living pain free.

Here are a few key points to consider:

  1. Improved Surgical Techniques: Advances in surgical techniques, such as minimally invasive procedures, have led to reduced trauma, faster recovery times, and decreased risk of complications for patients of all ages, including geriatric patients.
  2. Individualized Assessment: Each patient’s overall health, medical history, and specific spinal condition are taken into account when determining the suitability of surgery. Geriatric patients can undergo thorough evaluations to ensure they are physically capable of withstanding the surgery and subsequent rehabilitation.
  3. Pain Relief: The primary goal of spine surgery is often to alleviate pain and improve quality of life. For many geriatric patients, living with chronic pain can lead to decreased mobility, independence, and overall well-being. Successful surgery can provide significant relief from pain and enable these patients to engage in daily activities more comfortably.
  4. Multidisciplinary Approach: Geriatric patients often benefit from a multidisciplinary approach to care. This may involve collaboration between orthopedic surgeons, neurosurgeons, anesthesiologists, physical therapists, and other healthcare professionals to optimize surgical outcomes and postoperative recovery.
  5. Preoperative Preparation: Geriatric patients may require additional preoperative assessments and optimization to ensure they are in the best possible condition for surgery. This may include managing chronic medical conditions, addressing nutritional needs, and conducting thorough preoperative testing.
  6. Rehabilitation and Postoperative Care: A comprehensive rehabilitation plan is crucial for geriatric patients undergoing spine surgery. Physical therapy and guided exercises are essential for regaining strength, mobility, and functionality. Healthcare professionals will work closely with patients to ensure a successful recovery process.
  7. Life Expectancy: When considering surgery for geriatric patients, life expectancy is taken into account to assess the potential benefits of surgery over the patient’s remaining years. If surgery can significantly improve quality of life during this time, it may be a viable option.

It’s important to note that while spine surgery can provide relief for many geriatric patients, the decision to undergo surgery should be made in consultation with a qualified medical professional. Each patient’s individual health status and preferences play a significant role in determining the best course of action.
